This role focuses on providing compassionate care to the elderly population in both clinic and post-acute settings.Schedule: Full-Time, Day ShiftHours: Monday - Friday, 8:30 a.m. - 5:00 p.m. Position Highlights:•Full-time, day shift schedule•Monday through Friday, 8:30 AM - 5:00 PM•Occasional coverage at Stratford Pines (Midland, MI) Key Responsibilities:•Conduct visits for new patients•Rounding on established patients•Manage patient discharges•Collaborate with physicians and care teams to provide high-quality geriatric care Qualifications:•Graduate of an accredited Nurse Practitioner or Physician Assistant program•AHA-approved Basic Life Support (BLS) certification•Experience in geriatrics or primary care preferred, but new graduates will be consideredThis is a great opportunity for an APP looking to build meaningful relationships with patients and work within a supportive team dedicated to senior care in a welcoming community setting.
