Schedule: Full-Time Monday Friday Clinic Hours Close at 5:00 PM
Setting: 100% Outpatient No Nights No Weekends No Call
Patient Population: Primarily Geriatric / Medicare

An established outpatient primary care clinic on the South Side of Chicago is seeking a Board-Certified or Board-Eligible Internal Medicine or Family Medicine Physician to care for a predominantly geriatric, Medicare-focused patient population.

Role Overview
- Provide comprehensive outpatient primary care to older adults

- Collaborate closely with nursing staff, medical assistants, and care coordinators
- Practice within a value-based care model focused on outcomes, not volume
Qualifications
- MD or DO
- Board-Certified or Board-Eligible in Internal Medicine or Family Medicine
- Active Illinois medical license or ability to obtain
- Interest in outpatient, geriatric-focused primary care
