Perform and document Medicare Condition of Participation (CoP)-compliant medicationreviews for new hospice patient admissionsAttend hospice interdisciplinary team / group (IDT / IDG) meetings to discuss patients andprovide insights and guidance regarding the use of medications to palliate commonsymptoms experienced by hospice patientsParticipate in and provide clinical support to hospices by way of participation in Pharmacyand Therapeutic (P&T) committeesConduct and present comprehensive drug utilization evaluations which analyze medicationutilization data for compliance with Medicare CoPs and opportunities to reduce avoidablemedication costs via de-prescribing and therapeutic substitutionAuthorship / presentation of core OPPC clinical offerings, including webinars, companynewsletter, and symptom guide bookProvide evidence-based medication therapy advice during consults with hospice cliniciansUpdate hospice formularies with consideration of hospice-specific financial targets,medication pricing methods, and utilization patternsProvide clinical education and training to hospice clinicians and OPPC traineesProvide preceptorship to Advanced Pharmacy Practice Experience (APPE) pharmacystudentsMaintain appropriate records of consultations and meetingsTravel as necessary to provide local, regional, and national support to our clients, companypharmacies and sales and marketing efforts
