Fostering positive relationships between individuals served and their circle of support.Encouraging self-help activitiesImplements Individual Service Plan and Behavior Support PlansManage Individual's behaviors by prevention or de-escalation of aggression. Keeps accurate daily documentation of activities, fire/ disaster drills, Behavior Support Plans, medications, goal and data tracking, as well as other documentation.Transport individuals to and from community activities and medical appointments.Transporting clients or running errands for clientsOther duties as assigned.
