Job Function:
- Evening Shift.
- 3:30 PM - 11:00 PM.
- Provide direct patient care in accordance with established care plans and scope of practice.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patient vital signs, conditions, and responses to treatment.
- Report changes in patient status to the RN or physician in a timely manner.
- Assist with admissions, discharges, and coordination of care with the interdisciplinary team.
Job Requirements: Job Requirements: - Skilled Nursing Facility (SNF) experience preferred, but not required.
- Hospital experience will be considered.
- New graduate LPNs are eligible for full-time positions only.
- New graduate LPNs are not eligible for part-time roles.
