Licensed Practical Nurse
Reason: Replacement Department: Mohawk General Clinic Job Category: Direct Patient Care Job Title: Licensed Practical Nurse Duties: Responsible for assisting physicians with the evaluation and management of all members presenting to the Clinic. Conducts assessments, enters data into EMR, and assists in development of care plans and coordination of care. Assists in implementing physician orders and order follow up, medication refills, patient/family education, identifying opportunities for preventive screenings, clinical issues to evaluate all members, notification of member's test results. Coordinates front office team to schedule appointments, meetings. Communicate effectively to providers and all departments to resolve issues. Set up all members for procedures, exams, and obtain vital signs. Utilizes EMR (electronic medical record). Maintains supplies in exam room, and all other duties within the scope of practice as assigned by clinic providers. Clinic hours are Monday - Friday 6a-6p, Saturday 8a-12p. Training Monday through Friday 8a-5p. This is an onsite role.
Skills: At least one year experience in a Clinic or Physician's office. Strong phlebotomy skills Strong front office experience, including phones, scheduling, greeting patients. Computer skills including Electronic Health Record (EHR) system and Microsoft Office Exceptional customer service skills Ability to work within clinic hours Top 3 skills (non-negotiable) Computer skills, exceptional customer service, flexible This is an employer based clinic, the patients at this site primarily speak English and work in the office but there are occasional Spanish speaking patients. Approximately 25 patients (could be a couple more or less) – this includes lab and office visits. They will be doing lab draws, urine dipstick, point of care testing for flu, strep, COVID.
