Licensed Practical Nurse (LPN) – Continuing Care (Night Shift)
Location: Grand Rapids, MI 49503 (On-site)
Schedule: 12-hour nights, 7:00 PM–7:30 AM , every other weekend (no on-call)
Pay: $28-$34/hr
Scrubs: Navy Blue
Summary
Now hiring a Licensed Practical Nurse (LPN) for a Continuing Care / Long-Term Care setting in Grand Rapids, MI. This LPN role is focused on high-quality, patient-centered care with strong teamwork and consistent documentation. You’ll provide direct patient care under the direction of an RN, mid-level provider, or physician and work closely with the care team to support safe, timely, and respectful care.
Responsibilities
- Support patient assessment, care planning, implementation, and evaluation in coordination with RN/provider
- Provide direct patient care consistent with unit routines, policies, and procedures
- Communicate with patients/families regarding treatments, medications, and equipment (as appropriate and coordinated with RN/provider)
- Document care in the patient record (computerized and/or written) according to guidelines
- Collaborate with the healthcare team to ensure continuity of care and coordinated services
- Maintain a clean patient environment/work area; report equipment issues/malfunctions as needed
- Document chargeable items per standards
- Participate in quality monitoring and support safety/risk-reduction initiatives
- Follow privacy, confidentiality, and data security expectations; complete required competencies on time
- Perform other duties as assigned
Qualifications
- Active, current, unrestricted Michigan LPN license (required)
- High School Diploma/GED (required)
- Completion of an approved nursing program (required)
- Successful completion of a competency-based orientation program (required)
- BLS certification (American Heart Association or American Red Cross) within 90 days (required)
- Associate’s Degree from an approved nursing program (preferred)
- Prior healthcare experience (preferred)
