Horizon Family Medical Group is seeking a Nurse Practitioner (NP) to join a thriving nephrology practice in the beautiful Hudson Valley region of New York .

Coverage includes:
- New Windsor, NY (Outpatient Nephrology Clinic)
- Newburgh, NY (St. Luke s Cornwall Hospital Montefiore)
Scenic region with easy access to NYC, New Jersey, Connecticut, and Pennsylvania

Clinical Responsibilities
- Manage chronic kidney disease, hypertension, electrolyte disorders, and renal conditions
- Provide inpatient nephrology consultations and hospital rounding
- Conduct daily dialysis rounding (acute & chronic dialysis patients)
- Order and interpret labs, imaging, and diagnostics
- Develop individualized treatment plans
- Educate patients and families on kidney disease management and diet
- Collaborate with nephrologists, dialysis teams, and multidisciplinary staff
- Maintain accurate EMR documentation
- Participate in quality improvement initiatives

Qualifications
- Current New York State Nurse Practitioner license
- National certification (ANCC or AANP) required
- Master s or Doctorate in Nursing required
- Active DEA license required
- Minimum 2 years nephrology experience required
- Experience in both outpatient and inpatient nephrology care
- Dialysis management experience required
- Strong clinical reasoning and diagnostic skills
- Excellent communication and teamwork abilities
Preferred Qualifications
- Hospital rounding experience
- Dialysis procedure familiarity
- EMR experience
- BLS & ACLS certification
