Job Summary: As Support Coordinator, you will provide case management to an assigned caseload of individuals. You will help foster personal growth, encourage effective teamwork, and assist individuals in finding meaningful opportunities that align with their interests. Schedule: Monday through Friday, from 7:30 AM to 3:30 PM Wage Range: $18-$21 per hour As part of team, you will help people in the following areas: Provide case management to ensure services are initiated, delivered, and evaluated effectively Create and train others on individualized plans for each person on assigned caseload Deliver clear written and verbal communication across internal and external teams Encourage growth by fostering social skill development and positive workplace interactions Find meaningful opportunities that align with their interests and bring purpose to their daily lives Assist individuals with personal care needs (as needed) while upholding dignity and respect
