Pay Range: $100,000 - $110,000 per year based on experience
Job Description:
We are seeking a sophisticated, confident, and highly competent Registered Nurse Case Manager to join our boutique, concierge medical practice based in Beverly Hills, CA. Our low-volume, high-touch model of care provides personalized, best-in-class services to a discerning international clientele.
We specialize in rehabilitation medicine, longevity and wellness, integrative and functional medicine, and comprehensive healthcare coordination. Our patients retain us on an annual basis, and we pride ourselves on long-standing relationships, deeply personalized care, and exceptional responsiveness.
As the Nurse Case Manager, you will be responsible for the coordination and oversight of individualized care plans, ensuring the highest quality of care for a small number of VIP clients. You will work closely with our lead physician, interdisciplinary team, and an extensive international network of healthcare professionals.
Responsibilities Include:
· Serve as the primary point of contact and advocate for patient care, ensuring a seamless and proactive healthcare experience.
· Develop, implement, monitor, and revise individualized care plans in collaboration with the physician and other healthcare professionals.
· Identify, secure, and coordinate services, diagnostics, and consultations across specialties and global providers.
· Maintain comprehensive, up-to-date clinical documentation and medical records.
· Supervise and support the clinical support staff to ensure execution of the care plan meets the practice's high standards.
· Administer medications, perform IV therapy, and conduct blood draws (preferred but not required).
· Source, evaluate, and maintain relationships with external providers aligned with our practice's values and standards.
· Actively participate in interdisciplinary team meetings and serve as a resource to patients, families, and team members.
· Mentor, train, and onboard new or junior clinical team members as needed.
· Maintain accurate and timely communication with patients and care teams via phone, email, and EHR platforms.
· Ensure compliance with regulatory and professional standards.
· Support physician with administrative and clinical coordination, including lab follow-up, imaging, medication refills, and orders.
Requirements Include:
· Current California RN license in good standing.
· Minimum of 3 years of nurse case management experience, preferably in a concierge, home health, or private practice setting.
· Basic Life Support (BLS) certification required.
· Strong clinical skills, including medication administration
· IV therapy and phlebotomy skills are required.
· Proficiency with Epic is highly preferred,
· Excellent verbal and written communication skills.
· Exceptional organizational, time-management, and follow-through abilities.
· Discretion, tact, and professionalism in working with a high-profile clientele.
· Passion for and knowledge of integrative, regenerative, and functional medicine is a strong plus.
· Technologically savvy and comfortable with digital health tools.

Pay Details: $100,000.00 to $110,000.00 per year
