Job Type: Part Time - 14 hours per week
Facility: Out Patient Facility
Responsibilities
- Provide outpatient nephrology consultations and follow-up care
- Diagnose and manage chronic kidney disease and related renal conditions
- Develop individualized, evidence-based treatment plans
- Collaborate with primary care providers, nursing staff, and care coordination teams
- Ensure timely referrals and continuity of care
- Maintain accurate and timely medical documentation
- Participate in quality improvement initiatives
- Ensure compliance with federal, state, HRSA, and managed care guidelines
Qualifications
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in Nephrology
- Active, unrestricted New York State medical license
- Active DEA registration
- Hospital privileges (or ability to obtain)
- Commitment to patient-centered, culturally responsive care
- Strong communication and teamwork skills
